Injury Reports

Woodstock Little League places a high priority on the safety of all players. Board members and managers alike go to great lengths to eliminate safety hazards, unsafe practice conditions and situations that may result in player injury.

However, accidents resulting in injury do occur as an inherent part of this game. When they do, managers need to obtain as much information as possible on the incident and submit an Injury Report to the league Safety Officer.

The procedure that managers need to follow to submit an Injury report is:

1. Download the Injury Report Form, and
2. Submit the completed form to Scott Baier, Woodstock Little League’s Safety Officer
